Initial Assessment Form

You can now easily design your own health coach or personal training initial assessment form with your own brand and style. Change any aspect of the design including changing colors, backgrounds, adding pages and or questions. You will receive both a Canva template and a pdf version with client fillable form fields. This will allow your clients to complete the device online and send it back to you. If you choose to create your own design in Canva, you can contact me to receive an estimate to add fillable forms to your document.
